It says the mistake was by the designers. Not sure it took much design. More likely some manager threw it together. We should know who it was. If someone actually was paid to design it then they should return their fee and pay for the recasting
 
Yes, it's very bad.
My personal knowledge with working at a book publishers is that things should be checked at various stages, if it was the printers fault then they reprint at their cost but if it was the publisher's fault and they sent incorrect information then they would bear the cost.

The sign should have been checked for errors at various stages including before it was physically installed on the site!
 
Never heard of the Commonwealth Walkway Trust before. apparently they encourage people to walk and have planted these plaques at 2,000 points of significance in The Commonwealth, of which it seems only 4 are in England proper (though one in each of the 4 channel islands.). The blurb about the city on their website (https://www.commonwealthwalkway.com/) seems good, though it certainly ought to be as it is written by Carl Chinn. It looks like they are looking for sponsors for most of the plaques , including the Birmingham one, so maybe it has not been paid for yet
