Would anyone be able to improve this photo of my wife? She is aged three in the photo but can't remember where it was taken.

Unfortunately much of the definition has been lost, particularly at the side of the face. If it's there on the original perhaps you could try scanning again? This is the best I can do:
It looks to me as it the pic in post #1 has either had its format converted, or has been sharpened or contrast adjusted until there's very little left.
If the image was a scan of a photo it looks to me as if the scanner might have been set to 'black and white' rather than 'colour' or 'grey scale'. See the settings on my scanner below ... if I set to 'black and white' I get results similar to the original image.
