Forms for recording military information

Almost a Brummie
Over the years I have collected, from various sources, free on the internet , forms that I have used as an aid when collecting info on individuals and families. Like this one

I was wondering if anyone had come across any that you can use when researching military history?

For example I have found the following:

but this is aimed at American military records and I am really looking for something for the Boer War or WW1.

I am off to Kew in a couple of weeks and I thought a similar form would help to ensure i had covered everything.
