Birmingham Queens Hospital Chapel

I am trying to trace a mosaic memorial tablet depicting the good shepherd, dedicated in June 1918 to a nurse, Maude Amy Buckingham QAINMS.

The tablet was in the chapel of the Queens Hospital, the remaining buildings of which are part of student accomadation for Birmingham University. I have contacted the relevent people at the University. They have told me ,the chapel no longer exists and the memorial tablet appears not to be among those that survive and on display in the main building.

My question is, when was the chapel demolished and what would have happened to the plaque, does it still exist, if so where is it??
